In consideration for being permitted to participate in a California Athletic Soccer Club ("CASC") Soccer Camp, including all training sessions, scrimmages, games, competitions, conditioning activities, classroom sessions, and related camp activities (collectively, the "Camp"), I, the undersigned parent or legal guardian of the participant, acknowledge and agree to the following:

Assumption of Risk

I understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to:

  • Falls, slips, and collisions with other participants, coaches, spectators, goals, equipment, and facilities
  • Sprains, strains, fractures, dislocations, concussions, and other injuries
  • Heat-related illness, dehydration, sun exposure, weather-related conditions, and poor air quality
  • Exposure to uneven playing surfaces and field conditions
  • Illness, infection, or other medical complications
  • Permanent disability, paralysis, or death

I understand that these risks may arise from the participant's actions, the actions of others, field conditions, weather conditions, or the ordinary negligence of CASC and its representatives. I knowingly and voluntarily assume all risks, both known and unknown, associated with participation in the Camp.

Release of Liability

To the fullest extent permitted by law, I, on behalf of myself, my child, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless California Athletic Soccer Club (CASC), its officers, directors, employees, coaches, trainers, volunteers, independent contractors, sponsors, affiliated organizations, field owners, facility operators, and all other persons acting on their behalf (collectively, the "Releasees") from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or relating to participation in the Camp, including those resulting from the ordinary negligence of the Releasees.

This release does not apply to claims arising from gross negligence, reckless conduct, or willful misconduct to the extent such claims cannot be waived under California law.

Medical Authorization

I certify that my child is physically capable of participating in soccer activities and has no medical condition that would prevent safe participation except as disclosed below.

I authorize CASC staff, coaches, volunteers, and representatives to obtain emergency medical treatment for my child if I cannot be reached and immediate medical care is deemed necessary.

I authorize CASC staff to provide basic first aid, hydration assistance, ice treatment, and other non-emergency care deemed reasonably necessary for my child's health and safety during camp activities.

I understand that reasonable efforts will be made to contact the parent/guardian or emergency contact prior to treatment when circumstances permit.

I accept full responsibility for all medical expenses incurred as a result of participation in the Camp.

Please provide any medical conditions, allergies, medications, injuries, or other information that camp staff should be aware of:

Hydration, Weather, and Safety Policy

I understand that soccer camp activities may take place in outdoor environments where participants may be exposed to heat, sunlight, changing weather conditions, poor air quality, and other environmental factors.

Participants are required to arrive adequately hydrated, bring a refillable water bottle, wear appropriate athletic attire, and follow all hydration and safety instructions provided by camp staff.

I acknowledge that California Athletic Soccer Club (CASC) reserves the right to modify, shorten, postpone, suspend, relocate, or cancel any camp activity due to weather conditions, excessive heat, poor air quality, field conditions, facility issues, or any other circumstance that CASC determines may impact participant safety.

I understand that such modifications may occur with little or no advance notice and agree that decisions regarding participant safety shall be made solely by CASC staff and representatives. I further acknowledge that participation in outdoor athletic activities may increase the risk of dehydration, heat-related illness, sun exposure, and other environmental injuries, and I voluntarily assume such risks on behalf of my child.

Supervision and Transportation

I understand that CASC supervision is limited to designated camp hours and designated camp areas.

I acknowledge that I am responsible for arranging transportation to and from Camp and that CASC is not responsible for participants before official check-in or after official dismissal each day.

Participant Conduct

I understand that participants are expected to follow instructions from coaches and staff, act respectfully toward others, and comply with all camp rules and safety requirements.

CASC reserves the right to remove any participant whose behavior is unsafe, disruptive, or detrimental to the camp environment. No refund shall be required in the event of removal for disciplinary reasons.

Media Release

I grant California Athletic Soccer Club (CASC) permission to photograph, videotape, record, and otherwise capture the participant's image, likeness, voice, and participation during Camp activities.

I authorize CASC to use such photographs, videos, recordings, and likenesses in club publications, social media, marketing materials, websites, promotional materials, and other club-related communications without compensation or further approval.

Indemnification

I agree to indemnify and hold harmless the Releasees from any claims, damages, liabilities, costs, or expenses arising from my child's participation in the Camp to the fullest extent permitted by law.

Acknowledgment

I HAVE READ THIS LIABILITY WAIVER, ASSUMPTION OF RISK AGREEMENT, MEDICAL AUTHORIZATION, AND MEDIA RELEASE. I FULLY UNDERSTAND ITS TERMS AND UNDERSTAND THAT I AM GIVING UP CERTAIN LEGAL RIGHTS ON BEHALF OF MYSELF AND MY CHILD. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY.
