Tryout Liability and Medical Release Form 2025-2026

Liability and Medical Release, the parent/legal guardian of the registrant, a minor, acknowledge and agree that the registrant is voluntarily participating in a tryout session with California Athletic Soccer Club (CASC). I understand that participation in soccer and sports activities inherently carries a risk of physical injury, and in consideration of allowing the registrant to participate in tryouts, I hereby release, discharge, and indemnify California Athletic Soccer Club,
its owners, affiliated organizations, sponsors, employees, coaches, volunteers, and field/facility owners from any claims arising out of the registrant’s participation in the tryout sessions.

As the parent or legal guardian, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent(s).

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate in tryouts and related activities held by or associated with California Athletic Soccer Club and its affiliate members, the undersigned acknowledges, appreciates, and agrees that:

Participation in such activities inherently includes possible exposure to injury. While particular safety measures and personal discipline may reduce this risk, the risk of serious injury does exist.

I KNOWINGLY AND FREELY ASSUME ALL SUCH INHERENT RISKS, both known and unknown, EVEN IF ARISING FROM THE ACTIVE OR PASSIVE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.

I willingly agree to comply with all stated and customary terms and conditions for participation in tryouts. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Releasees, their officers, directors, officials, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct activities, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE ACTIVE OR PASSIVE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I agree to defend and indemnify RELEASEES for any claims, causes of action, or matters arising from or in any way connected to my participation in such tryouts.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR MINOR PARTICIPANTS (UNDER AGE 18) This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward, including the risks of participation and their personal responsibilities for adhering to the rules and regulations. Furthermore, my child/ward understands and accepts these risks and responsibilities. I, for myself, my spouse, and my child/ward, do consent and agree to the release provided above for all the Releasees and, for myself, my spouse, and child/ward, do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

The signed waiver/release should be kept on file by the sports organization for at least 7 years or longer if the participant has contracted a serious illness.
